How do I fill out my patient’s Overseas Medical Attestation report?

As a physician, what do I need to complete on my patient’s Overseas Medical report?
Fill out the section outlined in the green box below with the appropriate tests.
- Do not fill out the red section. This is for CPSA to attest your physician’s license.
- Inform the patient that they need to send the form to CPSA to have the report attested as proof that you hold a valid Alberta physician license. The patient can follow the instructions outlined in the patient FAQ here.

The submitted form must contain your signature (a digital signature is acceptable).
CPSA only reviews medical forms signed by a physician. CPSA’s role is to verify that the medical exam was performed by a physician licensed to practice in Alberta.
