Does your sponsored physician want to change their practice details?

If your sponsored physician and wants to change sponsors or other details of their practice, such as scope or adding or changing locations, you must complete the Change to Physician Sponsorship form.

Important note: Physicians must maintain their sponsorship once they join CPSA’s Provisional Register.

Does your sponsored physician want to change sponsors?

You and your sponsored physician must complete the following steps:

Current sponsor notifies CPSA

As the current sponsor, you must provide CPSA with a Change to Physician Sponsorship form indicating withdrawal of Sponsorship.

Sponsored physician applies for new position

The sponsored physician will apply for a sponsored position on DoctorJobsAlberta.com.

New sponsor notifies CPSA

Once the sponsored physician is selected by the new sponsor, the new sponsor must close the position on DoctorJobsAlberta.com, as well as inform CPSA.

New sponsor submits form

The new sponsor submits a separate Change to Physician Sponsorship form indicating the new clinic location and scope of practice.

Important notes: The new sponsor should not submit a Change to Physician Sponsorship form until they have a signed contract with the sponsored physician. We will reject the Change to Physician Sponsorship form if we receive it from the new sponsor prior to receiving the Change to Physician Sponsorship form indicating withdrawal from the current sponsor.
