About the standard
The Patient Record Content standard was last updated in 2016; however, an updated draft was shared for feedback during Consultation 32 (Jan. 6-Feb. 6, 2026).
In addition to the draft version shared during the previous consultation, this draft incorporates proposed amendments to expand documentation requirements in support of new legislative obligations related to dual practice, including documenting financial disclosures and distinguishing between insured and non-Plan services where appropriate.
What’s changed?
- Documentation requirements related to dual practice expanded in alignment with related legislation, including documentation of financial disclosures where applicable.
- Language to improve clarity regarding insured and non-Plan services, including added requirements to support proposed retirement of the standard.
- Stronger emphasis on supporting continuity of care while remaining compliant to new legislation.
Since this consultation includes more standards than usual, in an effort to make this easier, you can access a summary document in the resources below that identifies each proposed change alongside rationale of how the edits align with the legislative and regulatory requirements of Bills 11, 13 and 29.
