About the standard
The Continuity of Care standard was last updated in 2022. The proposed amendments clarify expectations for follow-up responsibilities, investigation management and care coordination to better support patients receiving care across multiple providers and practice settings.
What’s changed?
- Clarifies expectations for investigation management, including timely review of results and appropriate patient follow-up.
- Strengthens requirements for coordinated care within team-based practice environments.
- Clarifies responsibilities when transferring follow-up care to another healthcare provider.
- Reinforces expectations for after-hours care and temporary absences to support continuity of care.
