New and improved way to inform CPSA of updates to your personal and professional contact information

Did you know that you no longer need to submit a Notification of Change form to update your contact details with CPSA?
CPSA is committed to improving self-service options for all of our regulated members and has recently added a ‘manage’ feature to CPSA Portal’s “Profile” section. This feature allows you to update your own profile details, eliminating the need to notify us via the Notification of Change form.
What updates can I make through my CPSA Portal profile?
You do not need to submit the Notification of Change form to CPSA when updating the following personal and professional details in your Portal “Profile” section:
- General contact email
- Primary practice location information published on the CPSA Directory, including:
- Address
- Business phone number
- Business fax number
- Mailing address
- Home address
When do I need to use the Notification of Change form?
You must still submit a Notification of Change form to notify CPSA of any of the following:
- A legal change of your first or last name
- Retirement
- Voluntary erasure request
- Closing of a medical practice
