My medical clinic is moving to a new address. How do I update the clinic’s information on CPSA’s Medical Clinic Registry?

If you are moving, you must register the new location.

Log in to your clinic’s registry dashboard using your existing credentials and select “Register Another Clinic.”

Complete the Clinic Profile and Information Questionnaire with the details of the new location. You will also be asked to indicate whether the current location will remain open or close following the move.

Important: Reporting a clinic move through the Medical Clinic Registry does not update physician practice locations with CPSA. The physician registration system and the Medical Clinic Registry are maintained separately to protect the privacy and integrity of physician and clinic information. To update a physician’s practice location, submit a Notification of Change form through the CPSA Portal.
