What is an Overseas Medical Attestation report?

An Overseas Medical Attestation report is a form/certificate necessary for anyone intending to work overseas. This report is issued when applying for an overseas work visa and includes medical tests that must be conducted by a physician.

As a physician, what do I need to complete on my patient’s Overseas Medical report?

Fill out the section outlined in the green box below with the appropriate tests.

  • Do not fill out the red section. This is for CPSA to attest your physician’s license.
  • Inform the patient that they need to send the form to CPSA to have the report attested as proof that you hold a valid Alberta physician license. The patient can follow the instructions outlined here.
  • The submitted form must contain your signature (a digital signature is acceptable).

Note: CPSA only reviews medical forms signed by a physician. CPSA’s role is to verify that the medical exam was performed by a physician licensed to practice in Alberta.
