Filling out an Overseas Medical Attestation report
What is an Overseas Medical Attestation report?
An Overseas Medical Attestation report is a form/certificate necessary for anyone intending to work overseas. This report is issued when applying for an overseas work visa and includes medical tests that must be conducted by a physician.
As a physician, what do I need to complete on my patient’s Overseas Medical report?
Fill out the section outlined in the green box below with the appropriate tests.
- Do not fill out the red section. This is for CPSA to attest your physician’s license.
- Inform the patient that they need to send the form to CPSA to have the report attested as proof that you hold a valid Alberta physician license. The patient can follow the instructions outlined here.

The submitted form must contain your signature (a digital signature is acceptable).
CPSA only reviews medical forms signed by a physician. CPSA’s role is to verify that the medical exam was performed by a physician licensed to practice in Alberta.
