What is an Overseas Medical Attestation Report?

An Overseas Medical Attestation Report is a form/certificate necessary for anyone intending to work overseas. This report is issued when applying for an overseas work visa and includes medical tests that must be conducted by a physician.

What is needed on my Overseas Medical Attestation Report before I submit it to CPSA?

The submitted medical report must contain your physician’s signature (a digital signature is acceptable). Your physician can follow the instructions outlined here for filling out the report.

CPSA only reviews medical forms signed by your physician. It is not necessary to include additional pages. CPSA’s role is to verify that the medical exam was performed by a physician licensed to practice in Alberta.

How do I submit my Overseas Medical Attestation Report to CPSA?

Step 1: Provide your physician with medical report

• Your physician can follow the instructions outlined here.

Step 2: Email medical report to CPSA

After your physician has filled out the report, email the form to registration@cpsa.ab.ca. We will return the form to the email address from which the initial form was sent.

• Important: We do not accept documents or forms sent by mail or courier, so please do not send them this way.

Step 3: Upon receipt of medical report, CPSA will:

• Verify that the physician holds an active license to practice within Alberta.
• Affix our signature, stamp and seal the report.
• The medical report and documents will be returned via the provided email. CPSA aims to process the document and return it within 4 business days.

Step 4: Submit your completed medical report to the party requesting the form

Please note: If the electronic copy of your report is not accepted, email proof of the rejection to registration@cpsa.ab.ca and book an in-person appointment. In-person appointments can be booked for Tuesdays.

Additional information

• CPSA is unable to attest other documents outside of the Overseas Medical Attestation.
• There is no charge for this service.
