Retiring or withdrawing from practice? We still need your RIF

Even if you will not practise next year, we still need you to complete your 2022 Renewal Information Form (RIF) to ensure our records are up-to-date.
What you need to do:
- Before you can access your Renewal Information Form (RIF), you will need to complete the required Patient Relations: Part 2 training. Training is available via CPSA’s Physician Portal.
- Once this training is complete, you will see an active link to your Renewal Information Form (RIF).
- Fill in the first question regarding your status for 2022. When you indicate you are retiring or withdrawing from practice:
- Your form will be much shorter, only asking for necessary contact information
- You will not have to make payment at the end of the form
- We will not issue you a practice permit
Please make sure you give your patients ample notice and ensure there is follow up on any outstanding investigations.
